Job Description:

Plans, implements, renders and evaluates physical therapy services in the home care setting using professionally approved assessment and therapeutic skills and modalities in accordance with the scope of practice for a Licensed Physical Therapist in New York State.

Minimum or Preferred Qualifications:

Bachelor Degree in Physical Therapy. Experience: One (1) year experience in Physical Therapy. Credentials: Licensure to practice as a Physical Therapist in NYS. Valid NYS drivers license. Specific Skills: Verbal ability necessary to communicate with patients and others. Written communication abilities, to be able to prepare accurate, concise and meaningful reports. Able to delegate to Physical Therapy assistant and Home Health Aides.

Salary Range:

$40.80-$50.90 per hour Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
